I. General provisions

1. This Procedure for filling out the form “Application from an individual for registration with the tax authority” (hereinafter in this procedure - the Application) was developed in accordance with paragraph 5.1 of Article 84 of the Tax Code of the Russian Federation.

2. The general requirements for the procedure for filling out the Application are similar to the general requirements given in paragraphs 2 - 4 of the Procedure for filling out the form “Notification of the selection of a tax authority for registering a Russian organization at the location of one of its separate divisions located in one municipality, federal city values ​​of Moscow and St. Petersburg in the territories under the jurisdiction of different tax authorities" (Appendix No. 15).

II. Procedure for filling out page 001 Application

3. At the top of each page of the Application, an individual registered with the tax authority indicates the taxpayer identification number (hereinafter - TIN) in accordance with the document confirming registration with the tax authority (Certificate of registration with the tax authority, notification on registration with the tax authority, information from the Unified State Register of Taxpayers (USRN), received in the prescribed manner, a mark in the passport of a citizen of the Russian Federation).

4. In the field “tax authority code” the code of the tax authority at the place of residence (place of stay) of the individual to which the Application is submitted is indicated.

5. In the fields “Last name”, “First name”, “Patronymic” the surname, first name and patronymic of the individual are indicated in full, without abbreviations, in accordance with the identity document. If an individual does not have a middle name, the number “1” is entered in the field consisting of one acquaintance.

6. If the Application is sent by mail and (or) the accuracy and completeness of the information specified in the Application is confirmed by a representative of the individual, the number of sheets of the attachment to the Application is indicated.

7. In the section “I confirm the accuracy and completeness of the information specified in this Application”:

1) when indicating a person confirming the accuracy and completeness of the information specified in the Application, the corresponding number is entered in the field consisting of one acquaintance:

"5" - individual;

"6" - representative of an individual.

2) if the accuracy and completeness of the information is confirmed by the individual who submitted the Application, his personal signature is affixed in the place provided for signature, as well as the date of signing the application;

3) if the accuracy and completeness of the information is confirmed by a representative of an individual, in the field “last name, first name, patronymic of the representative in full” the last name, first name, patronymic (if any) of the representative of the individual is indicated line by line in accordance with the identity document, in the place reserved for signatures, the personal signature of the representative of the individual, the date of signing.

The TIN of the person confirming the accuracy and completeness of the information specified in the Application is indicated if he has a document confirming registration with the tax authority (Certificate of registration with the tax authority, a mark in the passport of a citizen of the Russian Federation), and the TIN is used along with with personal data;

4) in the “Contact telephone number” field, indicate the contact telephone number at which you can contact an individual (his representative) indicating the telephone codes required to provide telephone communication. The telephone number is indicated without spaces or dashes;

5) in the “E-mail” field, indicate the email address when the tax authorities interact with a person confirming the accuracy and completeness of the information specified in the Application in electronic form;

6) in the field “name of the document confirming the authority of the representative” the name of the document confirming the authority of the taxpayer’s representative is indicated;

8. The section “To be completed by a tax authority employee” contains information about the code for the method of submitting the Application (according to Appendix No. 1 to Appendix No. 15), the number of pages of the Application, the number of sheets of copies of documents attached to the Application, the date of its submission (reception), number, under with which the Application is registered, the surname and initials of the name and patronymic of the tax authority employee who accepted the Application, his signature.

9. The section “Information on the registration of an individual with the tax authority and (or) issuance of a Certificate of registration of an individual with the tax authority (Certificate)” is filled out by an employee of the tax authority and it indicates:

information about the position, surname and initials of the name and patronymic of the tax authority employee who registered the individual, TIN and the date of registration of the individual, the signature of the employee is affixed;

and (or) information about the position, surname and initials of the name and patronymic of the employee of the tax authority that issued the Certificate of Registration to an individual, details of the Certificate and the date of its issue, the signature of the employee is affixed;

information about the surname and initials of the name and patronymic of the individual (his representative) who received the Certificate, his signature is affixed.

II. Procedure for filling out page 002 Applications

10. At the top of page 002 of the Application, the surname and initials of the name and patronymic of the individual who submitted the Application with a request for registration with the tax authority at the place of residence (stay) or receipt of the Certificate are indicated.

11. If in the period after 01.09.1996 the last name, first name, patronymic changed, the last name and the year of their replacement are indicated.

12. In the “Gender” field, the gender of the individual is indicated by entering in a field consisting of one familiarity the number “1” if the gender is male, or the number “2” if the gender is female.

13. The date of birth is indicated in accordance with the entry in the applicant’s identity document.

14. The place of birth is indicated in strict accordance with the entries in the applicant’s identity document.

If an identification document is presented that does not contain information about the date and place of birth, the information is filled in on the basis of a birth certificate or other document containing such information.

15. The code of the type of identification document is indicated in accordance with the reference book “Types of documents proving the identity of a taxpayer” (Appendix No. 1 to this Procedure). The details of the identity document (series and number of the document, the name of the body and code of the unit that issued the document, and the date of its issue) are indicated in accordance with the details of the applicant’s identity document.

16. The “Citizenship” field, consisting of one acquaintance, is filled in by putting the appropriate number:

"1" - citizen;

"2" - stateless person.

17. In the “Country Code” field, indicate the numeric code of the country of which the applicant is a citizen. The country code is indicated according to the All-Russian Classifier of Countries of the World (OKSM). If the applicant does not have citizenship, the code of the country that issued the document proving his identity is indicated in the “Country Code” field.

18. The “Address in the Russian Federation” field, consisting of one acquaintance, is filled in by putting the corresponding number:

"1" - place of residence;

"2" - place of residence (if there is no place of residence).

The address (district, city, other settlement, street, number of the house, building, apartment) of the place of residence in the Russian Federation is indicated in accordance with the entry in the passport of a citizen of the Russian Federation, residence permit or document confirming registration at the place of residence (if not presented passport of a citizen of the Russian Federation, or another identity document). If there is no place of residence in the Russian Federation, the address of the place of stay is indicated in accordance with the entry in the document confirming registration at the place of stay. The address of residence (place of stay) is indicated with a postal code.

The digital code of the region is indicated in accordance with the reference book "Subjects of the Russian Federation" (Appendix No. 2 to Appendix No. 15).

When filling out information about the applicant’s place of residence (place of stay) for the cities of Moscow and St. Petersburg, the “District” and “City” fields are not filled in.

19. In the field “I confirm the accuracy and completeness of the information specified on this page,” the signature of the applicant or his representative is affixed.

III. Procedure for filling out page 003 Applications

20. At the top of page 003 of the Application, the surname and initials of the name and patronymic of the individual who submitted the Application for registration with the tax authority at the place of residence (place of stay) or receipt of the Certificate are indicated.

21. In information about a document confirming registration at the place of residence in the Russian Federation (for a foreign citizen or stateless person; for a citizen of Russia who has presented an identification document other than a passport of a citizen of the Russian Federation) or registration at the place of stay (in the absence of a place residence), indicate the type of document in accordance with the reference book “Types of documents proving the identity of a taxpayer” (Appendix No. 1 to this Procedure) and the details of the document (series and number of the document, name of the authority that issued the document and the date of its issue) in accordance with the specified document.

22. The date of registration at the address of the place of residence (place of stay) is indicated on the basis of an entry in the passport or a document confirming registration at the place of residence (if not a passport, but another identification document is presented), or a document confirming registration at the place of residence.

23. The field “Former address in the Russian Federation” is filled in similarly to paragraph 18 of this Procedure.

24. The date of registration at the address of the previous place of residence (place of stay) is indicated on the basis of the entry in the passport (a document confirming registration at the previous place of stay). If the applicant submits a document confirming registration at the place of residence that does not contain the date of registration at the previous place of residence, then the date of registration is recorded without documentary confirmation.

25. In the field “Country of permanent residence before arrival in the Russian Federation (for a foreign citizen and stateless person)” the numeric code of the country of permanent residence before arrival in the Russian Federation by foreign citizens and stateless persons according to OKSM is indicated.

26. How to fill it out correctly

Sheet 1

On the first page you need to indicate the applicant’s full name (as written in the passport), the code of the tax authority where the application is being submitted, the number of pages and sheets of attachments, and leave a note confirming the correctness of the information provided. The remainder of the first page will be completed by the tax officer.

Sheet 2

The second page contains the applicant’s passport details and information about his place of residence. Pay attention to the sample TIN application: if you change your last name after September 1, 1996, you should indicate the previous last name in the corresponding line of the application. Some difficulty when filling out the second page is usually caused by the need to indicate the code of the identity document (21 - passport) and the country code (643 - Russian Federation).

Sheet 3

On the third page you need to enter information about registration in Russia (that is, about a document confirming it). This section is mandatory for non-residents, stateless persons and citizens of the Russian Federation who have not presented a passport. You must indicate your previous and current place of residence, and in its absence, your place of stay.

General rules

Study the example of how to fill out an application for a TIN and remember some subtleties:

  1. The applicant's full name must be indicated at the top of each page and a signature must be placed at the bottom.
  2. Write dates only in numbers, and words - in printed capital letters (if filling out by hand) or use Courier New font (point size 16).
  3. The fields should be filled out from the first square or familiar place.
  4. Marks, corrections, damage when fastening sheets are unacceptable.
  5. Each page is printed on a new sheet of paper.

The application form for obtaining a TIN was developed by the Federal Tax Service of Russia and approved by Order No. YAK-7-6/488 dated August 11, 2011, the application has form 2.2-accounting and is called “Application of an individual for registration with the tax authority.”

In order to obtain a TIN, a citizen of the Russian Federation must fill out this application, the form of which can be downloaded below, and submit it to the branch of the Federal Tax Service of Russia at the place of residence.

Your application must be accompanied by your passport along with a copy, as well as a document confirming your place of residence, if it does not coincide with that indicated in your passport.
